NC CHAMP Survey Topics
NC CHAMP survey items are revised each year to meet the child health surveillance needs of North Carolina and pertain to a wide variety of health-related topics. The following lists survey topics included on previous NC CHAMP questionnaires. For those topics that are not included on the survey every year, the survey year is indicated. Topics that do not indicate a particular survey year were included on the questionnaire every year. Previous survey topics have included:
