County Health Data Book Preface The County Health Data Book was created by the project staff of the North Carolina Community Health Assessment Initiative (NC-CHAI) to provide communities with quantitative data to support community health assessments. The County Health Data Book includes a wide range of health-related county and state data in eight main areas: Population, Pregnancy/Live Births, Mortality, Morbidity, Health Resources/Programs, Education, Community Health, and Environmental Health. Guidelines and worksheets for summarizing and interpreting these data are presented in an accompanying publication entitled the Community Assessment Guide Book. In addition to these statistical aids, the Community Assessment Guide Book provides tools for collecting and evaluating public health programs and polices. The Community Assessment Guide Book can be considered a resource document or toolkit for county public health departments, Health Carolinians task forces, and other community agenices. Funded by grant U82/CCU417938-01 from the Centers for Disease Control and Prevention, the NC-CHAI is a joint project of the State Center for Health Statistics (SCHS) and the Office of Healthy Carolinians/Health Education (OHC/HE). The goal of this initiative is to build the capacity of communities to use data for public health program planning and policy making. To achieve this goal, the SCHS and the OHC/HE are working to connect and strengthen the assessment activities of county health departments, local Health Carolinians task forces, and other community organizations.
