You may utilize the following:

or call the following lines depending on your area of concern:

For Local Health Department HIPAA questions please call the DPH Local Health Department HIPAA Hotline at (919) 715-3358

For all other HIPAA questions or comments call the HIPAA for Public Health line at (919) 715-0411

A HIPAA coordinator will review your comment(s) or question(s) and attempt to answer in a timely manner.
